New Patient Information
Are you new to Covina Family Dental? Dr. Tran and his team are looking forward to providing you with care that is always comfortable, personalized, and reliably high in quality. Before your first scheduled visit, we would appreciate it if you read through the rest of the information on this page. We want every new patient to arrive feeling well-informed and confident about what’s to come.
Your first visit at Covina Family Dental will consist of a comprehensive oral examination by Dr. Tran and review of your treatment options. Often, treatment can be started and (in most cases) completed the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.
Please provide the following information at the time of your consultation:
- Any x-rays taken by a previous dentist, if applicable.
- A list of medications you are presently taking.
- If you have dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.
IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian.
Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).
Before we can start your first appointment, all of the necessary new patient paperwork will have to be completed. To save time on this process, patients can get a headstart by downloading the files through the link below, printing them out at home, and writing in their personal information whenever they have the time.
